Please use this form to submit your interest in representing your county at the Midlands Inter-county Masters competition on Sunday 15th November at Sandwell, Birmingham.
Completing this form will automatically notify your County's Masters' Rep. The information in this form will help them to select their team and complete their team sheet.
You do not have to complete the times section, however this will help your team manager to make the best selections.
